Student Selection Criteria

  • Eligible for DORS service
  • Receiving Special Education or 504 services
  • Need supports to seamlessly transition
  • Desire to participate

10th Grade (or 3 years prior to exit)

Service Outcomes

  • Enroll

Direct Services

  • Discovery process: Self-determination, self-advocacy instruction
  • Discovery process: Positive Personal Profile,career profile
  • Student-led IEP development
  • Family support and participation

All services are adjunt to school and academic preparation

11th Grade (or 2 years prior to exit)

Service Outcomes

  • DORS Counselor assigned to each student and opens case
  • Applications made for PSE to receive postschool supports from DDA or MH

Direct Services

  • Work based experiences

All services are adjunt to school and academic preparation

12th Grade or 1 year prior to exit

Service Outcomes

  • Paid employment prior to exit
  • Linkages to CRPs and DDA or MH for ongoing supports OR
  • Linkages to postsecondary education/trades training with disability campus services in place

Direct Services

  • Paid employment supports
  • Health and social linkages
  • Public benefits management

All services are adjunt to school and academic preparation

Post School Completion (2 years beyond high school)

Service Outcomes

  • Post school follow up

Direct Services

  • In paid employment receiving supports from CRP (if needed) OR
  • Enrolled post secondary education receiving supports from Disability campus services (as needed/as requested)
